Primary amyloid myelopathy is a rare neurological disorder characterized by the accumulation of amyloid proteins in the spinal cord. This deteriorating condition typically manifests with progressive sensory disturbance in the limbs, often accompanied by spasticity. In this case report, we present a subject with primary amyloid myelopathy who presented displaying an unusual clinical manifestation.

The patient's initial symptoms included marked proximal muscle weakness, along accompanied by urinary incontinence. Analysis of cerebrospinal fluid revealed elevated protein levels consistent with amyloid deposits. Magnetic resonance imaging (MRI) demonstrated characteristic lesions in the spinal cord consistent with primary amyloid myelopathy.

The management focused on supportive care, including physical therapy and medications to manage symptoms. However treatment, the patient's condition progressed steadily. This case underscores the importance of considering primary amyloid myelopathy in the differential diagnosis of patients presenting with progressive neurological symptoms, particularly those with unilateral weakness.

An Uncommon Presentation of Crohn's Disease: A Case Report

We present a unusual case of Crohn's disease in a 28-year-old female presenting with novel symptoms. The patient initially described signs of nausea, which were mistaken for a common gastrointestinalillness. Further investigation revealed colitis. The patient was treated with conventional treatment for Crohn's disease, including steroids. The response to treatment was positive.

This case demonstrates the necessity of a comprehensive assessment in patients with suspected Crohn's disease, as its manifestations can be varied.

Findings

A male/female patient presented to the clinic with a chief complaint/history of symptoms/presenting concern of progressive shortness of breath, cough/dyspnea/respiratory distress. The patient reported a gradual/sudden/insidious onset of symptoms/complaints/manifestations, worsening over the past few months/year/decade. On physical examination, auscultation/palpation/inspection revealed decreased breath sounds in the lower/upper/bilateral lung fields.

Further/Additional/Supplementary investigations included a chest X-ray which demonstrated diffuse/patchy/interstitial opacities consistent with pulmonary fibrosis. A high-resolution computed tomography (HRCT) scan of the chest confirmed the diagnosis, revealing honeycombing/fibrotic bands/reticular opacities.

The patient underwent further tests/evaluation/assessments to rule out other potential causes/contributing factors/underlying conditions of pulmonary fibrosis.

The child was a 5-year-old male/female, who arrived at the hospital after consuming a large amount of acetaminophen. Upon arrival, the child exhibited symptoms of abdominal pain and was revealed elevated liver enzymes.

Treatment modalities were initiated to attenuate liver damage, including administration of N-acetylcysteine and supportive care. The patient's condition remained stable over the following days, requiring intensive/critical care in the pediatric intensive care unit.

This instance highlights the seriousness of acetaminophen overdose in children and the importance/necessity prompt medical intervention to minimize potentially life-threatening consequences.
